DUTIES AND RESPONSIBILITIES (Which are not in any hierarchical order) 1.
Supports and administers care coordination services through identifying, establishing, and coordinating community resources to meet client needs, providing comprehensive information, and determining appropriate referrals.
2.
Develops and implements systems to connect residents to sources and opportunities around housing, education, employment, financial capability, health/mental health other community resources.
3.
Trains, supervises, and establishes work standards for assigned staff, including the community resource liaison, in support of assigned tasks, ensures performance standards are met; reviews work and provides assistance in complex and/or difficult situations; and takes disciplinary action as needed.
4.
Coordinates new and existing programs internally and with other City departments and governmental agencies, social service providers, and private community agencies to ensure maximum program benefits and to prevent duplication.

Assist the Department's leadership team with identifying and evaluating social service needs in the community, provides strategic guidance and oversight around service provision and resource allocation.
6.
Develops and maintains a resource database of housing and social service providers, services, information and activities; prepares informational material as needed.
7.
Establishes, maintains, and enhances cooperative relationships with City departments, non-profits, local service providers, civic groups; philanthropic organizations; and the homeless community.
8.
Develop written protocols and tracking tools to improve the delivery of services.
9.
Reviews and tracks referrals for assistance, ensuring the referrals are timely, complete and in compliance.
10.
Conducts training and presentations on available resources or changes in existing resources.
11.
Performs other related duties as assigned.
Salary Depending on qualifications the starting salary for this position will bein the range of (Pay Grade 55):
Minimum Range Midpoint Range $26.

Qualifications Bachelor'sdegree with a major in Public Health, Social Work, Human Services/SocialSciences, Healthcare or related field (education will beverified using the Education Requirements listed below) and three (3)years of progressively responsible experience in the development, delivery,monitoring, or evaluation of social service programs which must include one (1)years of experience working with resources and referrals to special needspopulations.
Acurrent and valid Florida driver's license is required.
For application purposes, a valid driver'slicense from any state (equivalent to a State of Florida Class E) may beutilized; with the ability to obtain the State of Florida driver's licensewithin thirty (30) days from date of employment.
Education Requirements:
To be considered valid (or verified) for position qualification purposes, and consistent with City Hiring Policy 4-5, a High School, GED or equivalent diploma, and all higher education degrees or certifications must have been issued by:
a) a public education institution in the State of Florida or any State or Territory of the United States; or b) an institution accredited by an accrediting organization that is recognized by the Florida Department of Education or the U.
S.
Department of Education; or c) an institution that is accredited by an organization that is recognized by the Council for Higher Education Accreditation (CHEA); or d) an institution whose degree or certification is accepted by institutions identified in either a), b), or c), above, for the purpose of admission to institutions identified in either a), b), or c) above.
International degrees will be evaluated to determine if they satisfy qualification requirements.

State of Emergency:
When normal operations of City departments are suspended or interrupted due to a declaration by the Mayor that a state of emergency exists, all employees will be deemed critical by the City.
For the preparation and/or continuation of emergency operations or for special work detail, employees deemed necessary shall be required to work.
Employees deemed critical by the City but not needed for the initial preparation and/or continuation of emergency operations or for special work detail may be directed not to report to work, and given paid administrative leave or they may be reassigned to another location that best meet the needs of the City.
