6K a year The Legal Aid Society of Palm Beach County's Public Guardianship Project provides guardianship services to indigent adjudicated incapacitated elderly without family or friends to assist them.
JOB SUMMARY Provide ongoing and complete case management for adjudicated, incapacitated persons of the Legal Aid Society of Palm Beach County's Public Guardianship Program (PGP).
Visit wards on a mandatory monthly basis and as needed, conduct assessments, planning of medical/benefits care and delivery of same, whether by direct intervention or indirect assignment to other personnel or referral agency, and document each visit in files.
Position requires extensive travel in Palm Beach County.
Work schedule is as follows:

ESSENTIAL DUTIES AND RESPONSIBILITIES Visits wards on a mandatory monthly and as needed basis, or as requested by the program manager.
Completes ward's initial, quarterly and annual plan.
Documents all visits, telephone calls and other ward activities in the ward information management system within one week following activity.
Accesses entitlement and/or financial assistance for the ward.
Ensures wards are receiving appropriate and necessary medical care, and basic needs are being met.
Ensures ward is in a safe, appropriate environment, free from potential harm.
Attends staff and other meetings in the community as assigned.
Maintains strict confidentiality of wards' personal and fiscal information.
Stays current on emerging professional information and incorporates into case management services on an ongoing basis.
Complies with all agency policies and procedures.
Works with other staff members to form a positive, supportive team atmosphere.
Must work the days and hours to perform all assigned responsibilities and tasks, and be punctual and timely in meeting all performance requirements, including but not limited to, attendance standards and work deadlines.
Ability to efficiently and effectively perform tasks independently with only moderate supervision.
Must maintain courteous, professional and effective working relationships with employees and clients of the organization.
Assists the program manager with special projects, assignments and any additional duties as deemed appropriate or as needed.
Documents mileage on a daily basis and ensures that mileage forms reflect the names of the wards visited.
Documents weekly schedule on the Outlook calendar and updates any changes as soon as possible.
Provides copies of visitation log, mileage form and Outlook calendar to the program manager at the beginning of each month, reflecting the prior month's activities.
POSITION REQUIREMENTS MSW required.
BA/BSW plus relevant adult welfare/social services experience may substitute for degree.
Minimum two years' recent experience with adults having physical and/or mental limitations.
Working knowledge of computer software such as basic Microsoft Office applications/programs (Word, Excel spreadsheet, WordPerfect) required.
Effective and excellent communication skills, both orally and in writing, bi-lingual, organizational and planning, problem-solving, and interpersonal skills required.
Ability to effectively communicate with wards, families and staff from community agencies and engender cooperation from same.
Crisis intervention, one-on-one counseling, and assessment skills required.
Nursing homes/facility operations knowledge required.
Ability to work independently, and sensitively and effectively handle difficult situations with individuals who lack capacity.
Must pass Level 2 background screening and internal conflict screening process.
Position requires extensive travel in Palm Beach, Martin and St.
Lucie Counties.
Must have a reliable vehicle, and requires maintaining a valid Florida State drivers' license with a satisfactory driving record and eligibility for coverage under any applicable agency insurance.
Occasional to frequent longer work hours required, including evening/weekend hours, and frequent visits to wards' homes which may be potentially hazardous environments.
Ability and willingness to participate in ongoing professional development activities in order to keep current on new developments in the field and to enhance skills already maintained.
Salary Range:
Salary depending on experience.
