Social Worker (HUD/VASH)

Duties may include, but are not limited to:
Provides consultation/education to veterans and families on community resources, advance directives and VA benefits. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Addresses Advance Directives and Organ Donation in accordance with the standards of their work area. Assists patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. Documents social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. Responsible for ensuring that homeless Veterans are considered as primary participants for program acceptance. Coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. Develops the screening procedures to involve a multidisciplinary team approach and is responsible to ensure criteria and procedures are maintained. Provides primary input for program participant selection based on structured interviews and clinical assessments. Regularly conducts and participants in outreach activities including field interviews, assessments and referrals for homeless Veterans contacted in the community, shelters, and on the streets, as well as those who are referred by VA residential. programs, medical centers or outpatient clinics. Maintains current and establishes new relationships with other community-based service providers for the homeless, and when appropriate, represents VA at community service provider meetings. Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. If appropriate, the incumbent provides referrals or consults, or in some cases direct admission of the Veterans prior to HUDNASH participation, for other VA programs including community-based residential treatment such as Grant and Per Diem (GPO) Programs, Health Care for Homeless Veterans (HCHV contract residential treatment, or Compensated Work Therapy (CWT) and Compensated Work Therapy Transitional Residence (CWT/TR) Programs. Makes routine visits to each Veteran in residence to assess their environment and provide on going clinical services. Devises methods of obtaining data and evaluates data to determine the effectiveness of treatment and identifies consistent qualities of successful and non successful treatment. Responsible for following local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for GARF and Joint Commission accreditation. Responsible for program oversight, monitoring and administration, serving as one of the primary contacts with local Public Housing Agencies (PHAs). Performs site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes. Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. Conduct psychosocial assessments to determine needs of veterans and family members. Participates in the treatment planning process with other disciplines. Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. Provides individual, family and group counseling as appropriate and within their Scope of Practice. Responsible for data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements. Responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. Maintains data and statistical compilations to comply with Joint Commission, CARF, VA and Medical Center policy and procedures. Supervises the practice of social work students, or a less experienced social worker. Participates in committee assignments, as deemed necessary by the service manager. Participates in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends. Cross trains to assist and provide cross coverage when needed within Social Work Service as well as collaborative duties as assigned. Work Schedule:

Citizenship. Be a citizen of the United States. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS-9 grade level. For the GS-11 grade level and above, the candidate must be licensed or certified. English Language Proficiency. In addition to the basic requirements for employment, the following criteria must also be met when determining the grade of candidates. Social Worker, GS-9 Experience, Education, and Licensure. None beyond the basic requirements. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. Ability to establish and maintain effective working relationships and communicate with clients, staff, and representatives of community agencies. Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Assignments. Individuals assigned as GS-9 grade level social workers are considered to be at the entry level and are closely supervised, as they are not yet functioning at the independent practice level conferred by independent licensure or certification. Social workers at the GS-9 entry level are typically assigned to program areas that do not require specialized knowledge or experience. Since social workers at this level are not practicing at an independent level, they should not be assigned to program areas where independent practice is required, such as in a CBOC, unless there is a licensed social worker in the program area who can provide supervision for practice. GS-9 social workers provide psychosocial services in the assigned area under supervision. Social Worker, GS-11 Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio- economic, cultural, ethnic, educational and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Assignments. This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. They are assigned to all program areas, including but not limited to:
inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Duties include but are not limited to:
assessing and documenting identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence. Independently assesses the psychosocial functioning and needs, identifying the strengths, weaknesses, coping skills, and psychosocial acuity. Identifies family/caregiver stressors, conducts assessment and provides specific interventions. Provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causation in differential diagnosis and treatment, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice. Links the Veteran with services, resources, and opportunities, in order to maximize independence, health, and well-being. Conducts timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate. Provides interventions independently with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems. Provide a range of interventions and treatment modalities which may include individual, group, and/or family counseling or psychotherapy. Independently formulate and implement a treatment plan including measurable, achievable goals identifying the Veterans' needs, strengths, weaknesses, coping skills, and psychosocial acuity. Traveling throughout the medical center and the community is required, as is performing activities involving sitting, walking, and standing, bending and carrying such items as books, papers, and files. May be necessary to travel into the community to conduct interviews.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
