Responsibilities:

This is a Health Program Educator A position responsible and accountable for the coordination, implementation, and evaluation of community-based health promotion, health education, and chronic disease prevention programs outlined in the department 's strategic plan.
Work is performed under the supervision of Public Health Service Manager F-SES.
This position requires independent judgment, initiative, applying general knowledge of business practices and skill in applying instructions to accomplish various job functions.
Responsible for the coordination, implementation and plan activities related to healthy eating and active living including, but not limited to strategies for worksites, schools, businesses, neighborhoods, and hospitals.
This position works with internal staff and a wide variety of traditional and nontraditional community partners to promote and improve the health of the community.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocols and Procedures.

Responsible for mobilizing, empowering, motivating, and educating stakeholders and communities about evidenced-based strategies and interventions, best practices, and changing social behaviors using community health assessment data and community health planning models.
Works with community groups, coalitions , and advisory groups interested in reducing the burden of illnesses, disabilities, and premature death associated with unhealthy lifestyles choices and behaviors that will improve the overall health, wellness, and quality of life across all life stages.

Serves as the liaison with central office for direction for the promotion and implementation of initiatives such as Healthiest Weight FL and required work plan activities focused on healthy eating and active living.

Provides technical assistance to the community by initiating and supporting health promotion programs, initiatives, activities, and campaigns that will increase personal and social responsibilities; modify unhealthy behaviors and choices; and help to initiate, expand or sustain evidenced-based strategies, interventions, and best practices.

Serves as a resource person to community partners, businesses, worksites, schools, community groups, and others by responding to requests for data, trainings, meetings, targeted outreach activities, and community presentations.

Provides technical assistance and guidance to the community through effective teamwork and collaborative efforts, using available resources.

Facilitates and assists with facilitating trainings, meetings, presentations, and workshops on a wide variety of health promotion and health education programs, strategies, interventions, and best practices to improve the health of the community.

Continuously seeks innovative ways to improve the health of the community by meeting with key agency staff and community partners to identify resources and reach consensus on partnership roles and responsibilities.

Implements and evaluates health promotion strategies and interventions with the support of community partners and makes recommendations on continuous improvement.

Programmatic Support Activities:

Completes, collects, and analyzes community assessment data when necessary.
Provides assistance and support to management and community partners working on common goals and priorities.

Develops programmatic action plans with a multiplicity of community partners for implementation and evaluation to address the health needs, problems at the community and organizational level.

Tracks and monitors programmatic activities to meet measurable objectives and outcomes of the program work.
Attends and completed required trainings.
Participates on agency teams meeting as needed.

Perform other related duties as assigned.

Required Knowledge, Skills, and Abilities:

  • Knowledge of the principles and practices of health education.
  • Knowledge of a variety of health specialty areas.
  • Knowledge of the methods of data collection.
  • Ability to plan, implement, coordinate, and evaluate community-based health promotion, health education, and/or chronic disease prevention programs.
  • Ability to collect and analyze data.
  • Ability to evaluate health education practices for compliance with applicable statutes, rules and regulations.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to understand and apply applicable rules, regulations, policies and procedures.
  • Knowledge in community health planning.
  • Knowledge and experience in community mobilization and engagement; collective impact strategies and techniques; and partnership development and sustainability.
  • Ability to use Microsoft applications such as Outlook, Excel, Word, and PowerPoint.

  • Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency.
    Emergency duty required of the incumbent includes working in special needs shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
